Event Request & Graphic Request Form
Purpose of the Form: The following forms have been designed to streamline the process of requesting events and graphic designs. Providing clear and detailed information will help ensure your request is processed efficiently and our team is able to assess and hopefully accommodate your needs.
Please submit this form for any event you plan to host at The City of Zion Church that requires promotion by The City of Zion Church. Dates, room reservations, tech needs, and communication needs will have to be approved before an event is officially put on the calendar and a communications plan created. This form will go through the staff team and must be submitted at least six (6) weeks before an event to give adequate time for planning communications and calendaring. Please note that submitting the form does not automatically approve your request. Once the request has been evaluated, you will receive an email confirmation within one week that your event has been booked, or if additional information or change is needed, you will be contacted by a staff member. Please complete this form in its entirety so that we may ensure that all details of your GRAPHIC DESIGN are included. Please complete and submit your request at least two weeks in advance to allow sufficient time for processing, reviewing, preparing, and approving. All WEEKLY OBSERVATION requests MUST be submitted 14 days prior to Sunday morning worship by Wednesday close of business (4 p.m.) for display.
